MEDICAL CONCERNS

ABSENCES:

If your child is out of school for more than three (3) days due to a medical condition, please submit a physician's note to the Nurse's Office, or the Attendence Secretary.

ASSISTIVE DEVICES:

Students who have a fracture or sprain requiring assistive devices such as: crutches, wheelchair, cast, or boot are required to have a doctor's note. The note needs to outline all physical education, and dance class restrictions including a timeline.

BUILDING EVACUATIONS:

The nurse will initiate an evacuation plan to staff members indicating the student's needs.
